Include your e-mail address with your registration. You will be sent an e-mail confirmation when your registration form and payment has been received. Please print this e-mail and bring it with you to pick up your name badge on-site.

REGISTRATION CATEGORIES
Attendies who are celiacs or family members of celiacs, and teens should register for the first day of the conference only, Tuesday, March 20, 2007. Use the PATIENT EDUCATION DAY registration form.
Dietitians, Nurses, Social Workers and other health professionals may register for either or both days for continuing education credit. Patient Education Day focuses on the practical applications; Physician Education Day focuses on the clinical and pathological applications.
Doctors, Dentists and Pharmacists should register for the second day of the conference for CME credits, Wednesday, March 21, 2007. Use the PHYSICIAN EDUCATION DAY registration form.
ATTENDANCE FEES
Patients and their families attending on Tuesday, March 20th -- $30 per person
Late fee for persons registering after February 20th -- $5.00
Nurse pracitioners, dietitians, diatetic technicians,and physician assistants
$100 per day for continuing education credit.
Late fee for nurses, dietitians, diatetic technicians and physician assistants registering after Feberuary 20th -- $10.00
Social Workers pay $35 per day for continuing education credit.
Doctors and Pharmacists attending on Wednesday, March 21st -- $250 for CME credits
Late fee for doctors and pharmacists registering after February 20th -- $25.00
Dentists attending the dental portion of the program on Wednesday, March 21st -- $50 for 1 CME credit
Late fee for dentists registering after February 20th --$10.00

PATIENT ~ TEEN EDUCATION DAY

This day is designed for patients and their families, dieticians, nurse practitioners, physician’s assistants and others in the healthcare arena who wish to enhance their knowledge of the most current research and how to cope with the gluten-free diet.
This is an interactive, educational program that is designed for teens that are 8 -18 years of age and have been diagnosed with celiac disease or are a first or second degree relative of someone who has been diagnosed with celiac disease. The educational sessions will provide an opportunity for teens to network and share experiences with other teens living with celiac disease.

PHYSICIAN ~~ DENTIST ~~ PHARMACIST EDUCATION DAY

The program that will be presented on this day is a scientific program designed for physicians and medical professionals. Experts will discuss the latest information on the epidemiology, clinical manifestations, diagnosis, pathogenesis, the impact of the diagnosis of celiac disease, and therapy, both current and future, of celiac disease. This program is AMA approved with 1 CME credit for each 60 minute session attended.
ACCREDITATION: NEOUCOM is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. The American College of Nurse Practitioners (ACNP) and The American Academy of Physician Assistants (AAPA) accept AMA/PRA 'category 1' credit from organizations accredited by the ACCME. The American Nurses Credentialing Center (ANCC) accepts AMA/PRA 'category 1' credit toward recertification requirements.

GENERAL INFORMATION
Name badges must be worn at all times during the conference. Name badges may not be shared. Name badges will NOT be mailed in advance. Plan to pick up your name badge at the registration counter at NEOUCOM the first day of the conference.
Photo ID (driver’s license, passport or government issued photo ID) will be required when you check in at the conference.
